Give Yoga A Try!

By no means am I an expert, but I have enjoyed yoga off and on for many years. I love how there are so many different styles and philosophies, to fit whatever mood you might be in. I personally prefer the ‘less serious’ classes…one in particular I like is taught by a guy that makes fun of us and swears (“No, your other god damn right!”), ensuring that we get a good laugh in during class. The more serious ones are certainly great too, but I’m not flexible enough to take them. 🙂 

During cancer treatments, grieving, or any stressful time, taking ten minutes to an hour to relax and focus on yourself can be very rewarding. Most people are familiar with the physical benefits of yoga, such as increased flexibility, core strength, and energy, but what I appreciate most are the mental rewards I incur. Apart from a runner’s high, there is no calm like the calm after a session of yoga, whether it be a power vinyasa class or a calming restorative class. I may sometimes struggle to convince myself to get on the mat, but I never regret it once it’s over.

I encourage everyone to try yoga, even if you’ve tried it before and haven’t enjoyed it. Try a different style, a different teacher, or a different studio…there can be such differences between classes!

Something I’ve been very happy with is the advent of online yoga classes. This can be great when you’re not feeling up to leaving the house, or if you want to save some cash. Those studio classes can be very expensive and there are some great online alternatives. My personal favorite is yogaglo.com. For $18/month, you get access to an unlimited number of streaming classes taught by a variety of teachers in a variety of styles. What sets yogaglo apart from other online yoga sites for me is the ease of searching for classes. You can enter in that you want a 15-minute beginner class with a focus on opening your hips and you will likely be met with multiple results. It is very user-friendly and they are always adding new videos.

Another great web resource is ekhartyoga.com. I find Esther’s classes to be very restorative and calming and they are my go-to when I’ve had a rough day. She also has a subscription service that is very affordable ($12.50/month).

Managing Cancer-Related Fatigue

 In my previous post, I talked about some common contributors to cancer-related fatigue (CRF), a prevalent symptom among those with cancer and those undergoing cancer treatments. Somewhat paradoxically, studies support moderate exercise to help manage cancer-related fatigue. Upon closer inspection, this makes sense. The negative physiological processes associated with cancer and cancer-treatments, as well as general deconditioning caused by decreased physical activity from not feeling well, lead to a person having less ability to perform physical tasks that once may have been simple. So it makes sense that moderate exercise can help you feel a little better and make everyday tasks less taxing.

Here are some guidelines when figuring out what moderate exercise is best for you:

  1. Make it something you enjoy (duh). Now is not the time to punish yourself with an activity that doesn’t speak to you or to push yourself to your limits by training for that marathon you’ve always felt like you should do.
  2. Listen to your body and be kind to yourself. If you are an athlete, this may be especially hard. But this is not a “no pain, no gain” situation. If you love running, go ahead and try it, but don’t beat yourself up if your usual 7-minute per mile pace becomes a gentle jog interspersed with walking. Furthermore, a ten minute gentle walk is a great achievement…an hour long sweat-fest that makes you feel utterly exhausted afterwards is not what we’re after.
  3. Pace yourself. And I’m not just talking about with exercise. Be mindful of pacing yourself with your normal, daily activities. Rather than planning on getting ready in the morning, then running errands, then meeting up with a friend for coffee, give yourself time between activities to recuperate. And try planning your most strenuous activities early in the day, when you’re likely to be more rested.

I know this mostly sounds like common sense, but I think that when you’re not really feeling well, it’s easy to be hard on yourself.  Sometimes you need to hear from someone else to take it easy. Cancer-Related Fatigue

Cancer-related fatigue…what is it?

I’m a physical therapist at a large, urban hospital, and while far from an expert in the field, I often see patients who have been affected by cancer.  Some I see just after they’ve been diagnosed and have just undergone a significant surgery to remove a tumor before starting chemo or radiation.  Others I see during their cancer treatments, who happen to be in the hospital for a completely unrelated reason.  Still others I see long after they have been diagnosed and treated, but who may still carry deficits from their treatments or surgeries.

The first topic I wanted to discuss is cancer-related fatigue (CRF). It is very common and studies have found that it affects 59% to 100% of patients with cancer.  This is fatigue that is out of proportion to what you’d expect from doing a certain activity.  Something that may have been easy before being diagnosed or before starting chemo treatments, might now be very difficult and taxing.

The causes are not clearly understood, but can be attributed to many different influences, which I would like to outline in this post. I will follow up soon with a post on strategies for combating this fatigue.  Here are some of the things that can cause cancer-related fatigue…we call them contributors in the medical world.

Therapeutic contributors (ie, side effects of the cancer treatments):

  1. chemotherapy: symptoms can last 3-4 weeks after treatment stops, if not longer
  2. radiation therapy: can cause fatigue that increases with each subsequent treatment; again, can last at least 3-4 weeks after treatment stops
  3. bone marrow transplant: can cause fatigue that lasts up to a year after initial treatment
  4. biological therapy

Other contributors: 

  1. anemia: certain blood counts, such as a person’s hemoglobin or hematocrit, may be decreased by chemotherapy, which can cause feelings of fatigue; 7/10 people undergoing chemotherapy may develop this
  2. combination therapy: undergoing more than one treatment at the same time (ex., chemo and radiation) may increase CRF
  3. the tumor itself: the tumor takes nutrients from the body, at the expense of normal, healthy cells; this can result in weight loss, decreased appetite, increased fatigue
  4. decreased nutrition: chemo and radiation can have side effects of nausea, vomiting, mouth sores, taste changes, heart burn, and diarrhea, making it difficult to maintain proper nutrition during this time
  5. hypothyroidism: can occur if lymph nodes around the neck are undergoing treatment; having low thyroid function can increase fatigue
  6. medications: some medications used to treat the side effects of treatments can increase fatigue
  7. stress, pain, insomnia, depression: all of these can be related and contribute to overall feelings of exhaustion or fatigue
  8. attempting to maintain a “normal” lifestyle: by trying to keep up the same pace a person was enjoying prior to diagnosis or treatment, they may be overexerting themselves and increasing their fatigue levels

The point I want to stress is that cancer-related fatigue is a very normal and common condition (up to 100% of patients with cancer report experiencing cancer-related fatigue).  There are even strategies that can be implemented to address cancer-related fatigue, which I will get into with my next post!

I’m currently a physical therapist at a large, urban hospital in St. Louis. I see a broad range of patients, including some that have been affected by cancer and other potentially long-term illnesses. I hope to add some informative posts addressing common medical issues among this population into the mix.
